Job Summary

To support a mission-driven interdisciplinary clinical team, the full-time North Carolina Licensed NP/PA will conduct telehealth video visits, perform comprehensive assessments, and emphasize preventative care while collaborating with various healthcare professionals in a fully remote environment.

Key responsibilities

• Conduct primarily telehealth video visits and occasional in-home visits to provide comprehensive assessments and evidence-based care
• Identify care gaps and coordinate with care team members to develop individualized, whole-person care plans
• Utilize integrated technology to enhance member interaction and reduce administrative burdens

Required qualifications

• Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license
• 3+ years of outpatient clinical practice, preferably in primary care, family medicine, internal medicine, or geriatrics
• Active BLS certification at time of hire
• Comfort with delivering care via video telehealth and using electronic documentation systems
• Willingness to obtain additional licenses if necessary within the first 90 days
