Job Summary
To support an integrated care model, the remote North Carolina Licensed Social Worker will conduct comprehensive psychosocial assessments, develop person-centered care plans, and provide case management interventions for patients and families, collaborating with an interdisciplinary care team.
Key responsibilities
- Conduct comprehensive psychosocial assessments of patients and families
- Develop and implement person-centered care plan goals in collaboration with patients and caregivers

Required qualifications
- Master's Degree (MSW) in Social Work
- Current unrestricted LCSW or LICSW license in the state of North Carolina
- 2+ years of social work experience (internship not included)
