Key Responsibilities

Required Qualifications

• Must be a Physician
• Must have at least 2 years of clinical experience with prescribing controlled substances
• Must possess a valid DEA
• Must hold a valid multi-state license
• Mental health experience of 2 years is required; mental health certification is preferred
