Nutritional Support Coordinator
Job Summary
To support patients with chronic conditions, the full-time Nutritional Support Coordinator will provide remote nutritional education, conduct outreach, and coordinate care while managing a panel of 300-350 patients.
Key responsibilities
- Manage monthly nutritional outreach and support through daily phone calls with patients, providing tailored health coaching
- Educate patients on nutrition concepts and dietary modifications, reinforcing adherence to care plans
- Collaborate with clinical teams to address nutritional needs and escalate patient concerns as necessary
Required qualifications
- A Degree in Dietetics or Nutrition
- 2+ years of experience in clinical nutrition counseling or dietary care coordination
- Experience supporting patients with chronic conditions in various settings
- 2+ years of Electronic Medical Records (EMR) experience
- HIPAA-compliant home office setup with minimum internet speed requirements
