Ohio Licensed Care Management Associate

Job Summary

To support the OhioRISE Care Coordination department, the full-time Ohio Licensed Care Management Associate will manage inbound inquiries, process referrals, verify Medicaid eligibility, and complete Health Risk Assessments in a remote environment.

Key responsibilities

• Act as the first point of contact for members, providers, guardians, and community partners
• Process referrals and maintain accurate documentation within electronic record systems
• Support care coordination activities while ensuring HIPAA compliance and delivering compassionate customer service

Required qualifications

• High School Diploma or equivalent
• Minimum of 1 year of customer service experience in a healthcare environment
• Experience in care coordination or case management support
• Proficiency in Microsoft Excel and experience with electronic record systems
• Strong organizational and time management skills
