Job Summary
To support a growing telehealth initiative, the full-time Ohio Licensed Nurse Practitioner will manage clinical triage for patients requiring evaluation and treatment, acting as a liaison between triage teams and local clinical teams while working remotely.
Key responsibilities
- Act as a clinical resource for triage teams, evaluating and treating patients via telehealth
- Coordinate follow-up visits and updates between central triage resources and local clinical teams
- Address time-sensitive medication refills, lab results, and assist in service recovery for patients
Required qualifications
- Must be a certified Nurse Practitioner with an active license in Ohio
- Must have an active DEA license
- Demonstrated experience with telehealth and coordinating care virtually
- Ability to react decisively in urgent situations
- Strong organizational skills and experience with multi-tasking
