Job Summary
To support a growing pediatric practice, the full-time Ohio Licensed RN Care Manager will provide comprehensive, family-centered care management services to pediatric and young adult patients with complex medical, behavioral, and social needs, working remotely.
Key responsibilities
- Conduct comprehensive, trauma-informed assessments of patients, identifying high-risk and complex cases
- Develop individualized, evidence-based care plans in collaboration with interdisciplinary teams and community partners
- Monitor healthcare utilization and patient progress, revising care plans as needed to support successful outcomes
Required qualifications
- Bachelor of Science in Nursing from an accredited program
- Current, unrestricted RN license in Ohio; may be obtained within 90 days of hire
- Current Basic Life Support (BLS) certification from the American Heart Association
- Minimum of 3 years of clinical nursing experience; pediatric and managed care experience preferred
- Proficiency in MS Office and EMR software, such as Epic
