Job Summary
Seeking a full-time Ohio Licensed Telemedicine Provider (NP or PA) to deliver remote care to patients throughout Ohio, focusing on hormone replacement therapy, medical weight management, and preventive healthcare.
Key responsibilities:
- Evaluate, treat, and manage patients through individualized hormone optimization and preventive healthcare
- Maintain a balanced patient population with a focus on both men's and women's hormone replacement therapy
- Complete the hiring and credentialing process promptly to begin patient care with minimal delay
Required qualifications:
- Active Ohio Family Nurse Practitioner (FNP) license or Physician Assistant license with appropriate NPI
- Current DEA registration appropriate for practice in Ohio at the time of application
- Current National Provider Identifier (NPI) number without specialty
- Current national board certification applicable to the profession
- Experience or strong interest in hormone replacement therapy, preventive medicine, or medical weight management is preferred
