Job Summary
To support post-acute communities and healthcare providers across Oklahoma, the part-time Weekend Oklahoma Licensed Post-Acute Liaison will conduct remote assessments and pre-admission screenings while collaborating with facility Admissions Coordinators to ensure a smooth admission experience.
Key responsibilities
- Conduct pre-admission screenings and assessments to determine clinical, equipment, and transportation needs
- Develop and maintain strong referral development relationships with referral sources
- Maintain accurate records of daily activity and referral source interactions within the referral tracking system
Required qualifications
- Current and unencumbered Oklahoma LPN or RN license
- Experience as a hospital liaison
- Experience in a skilled nursing or skilled rehabilitation setting
