Oklahoma RN Case Manager

Job Summary

Joining a dedicated team, the remote full-time Oklahoma RN Case Manager will manage utilization review processes, coordinate patient care transitions, and develop discharge plans while working days and every other weekend.

Key responsibilities

• Completes comprehensive assessments of patients' clinical, psychological, and financial needs
• Recommends and coordinates timely transfers to appropriate levels of care based on clinical needs
• Communicates discharge care plans and collaborates with healthcare professionals to facilitate timely patient discharge

Required qualifications

• 2 years of experience in clinical settings such as home health, inpatient, or clinics
• Current RN licensure in the State of Oklahoma or a multistate license from an eNLC member state
• BLS certification from the American Red Cross or American Heart Association within 30 days of hire
• Ability to operate a vehicle and possess a valid Oklahoma State Driver's License
• Experience with managed care and payer/provider requirements is preferred
