Oregon Licensed MTM Pharmacist
Job Summary
To support a clinical engagement team, the contract MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and provide patient education while working remotely in Oregon with flexible hours between 8:00am and 8:00pm CST.
Key responsibilities:
- Engage members telephonically and schedule pharmacist consultation appointments
- Utilize prescription claims data to generate clinical recommendations and assess medication regimens
- Document patient care notes and participate in departmental meetings to enhance clinical programs
Required qualifications:
- Current and unrestricted Pharmacist license in the state of residence
- Strong knowledge of disease state management guidelines and MTM services
- 1+ years of experience working remotely within a cross-functional team environment
- Intermediate proficiency with Microsoft Word, Outlook, and Excel
- Bachelor's Degree or Pharm D
