Oregon Licensed MTM Pharmacist

Job Summary

To support a dynamic Clinical Engagement Services Center, the contract-to-potential-FTE Oregon Licensed MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and provide patient education while working remotely with flexible hours.

Key responsibilities:

• Engage members telephonically to schedule consultations and provide clinical recommendations based on medication therapy management protocols
• Ensure patient privacy and confidentiality by adhering to HIPAA regulations and organizational policies
• Document patient care notes and recommendations accurately and participate in departmental meetings to enhance clinical programs

Required qualifications:

• Current and unrestricted Pharmacist license in the state of residence
• Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services
• 1+ years of experience working remotely within a cross-functional team environment
• Intermediate proficiency with Microsoft Word, Outlook, and Excel
• Minimum education of a Bachelor's Degree or Pharm D
