Job Summary

To support patients on their musculoskeletal care journey, the full-time Oregon Licensed Physical Therapist Care Navigator will triage patients, guide clinical decision-making, and ensure access to appropriate care through high-touch telephonic engagement in a remote setting.

Key responsibilities

• Guide clinical decision-making by conducting assessments and determining appropriate care pathways for patients
• Build trusted relationships with patients through telephonic engagement and provide ongoing support to navigate their care journey
• Coordinate care across the healthcare ecosystem by collaborating with patients, providers, and caregivers to ensure continuity of care

Required qualifications

• Doctor of Physical Therapy (DPT), Doctor of Physiotherapy, or Master of Physical Therapy (MPT)
• Active, unrestricted Physical Therapist license in Oregon, with the ability to obtain additional state licensure as needed
• 5+ years of orthopedic or musculoskeletal clinical experience
• Strong clinical judgment and triage skills for assessing complex patient situations
• Experience in motivational interviewing and patient coaching to support behavior change
