Job Summary
To support the ACCESS Team, the full-time remote Outreach Specialist will drive patient participation and enrollment in the Aledade Access Program by conducting multimodal outreach, assisting with device onboarding, and managing operational queues to ensure seamless patient-centered care.
Key responsibilities
- Conduct outbound and inbound outreach calls to assist patients when automated workflows require manual intervention
- Manage high-volume patient inquiries and provide support for onboarding and clinical metric collection
- Accurately document patient interactions and track engagement metrics to refine operational strategies
Required qualifications
- High school diploma or equivalent
- Minimum of 2 years of experience in healthcare outreach, care coordination, or telephonic patient engagement
- Proven ability to meet performance metrics in a high-volume environment
- Regular and predictable attendance
- Ability to work shifts between 8:00 AM - 8:00 PM EST, including evenings and Saturdays as needed
