PA and NJ Licensed Clinical Care Manager

Job Summary

To support high-risk patient populations, the full-time PA and NJ Licensed Clinical Care Manager will collaborate with healthcare teams to design and implement care management plans, conduct patient assessments, and provide education and support, with the flexibility of remote work options.

Key responsibilities

• Designs and implements care management support activities for patients with chronic diseases
• Collaborates with physicians and staff to identify patients for care management and develops care plans
• Triages patient calls and provides consultation on symptom management and emergency situations

Required qualifications

• Associates Degree in an NLN-approved school of nursing required
• Bachelor's Degree in nursing preferred
• 1+ years prior nursing experience in internal/family medicine and care management preferred
• Active PA or NJ RN license required, with dual licensure within 3 months of hire
