PAC Engagement Specialist

Job Summary

To enhance engagement in post-acute care workflows, the full-time remote PAC Engagement Specialist will manage client outreach, training, and data monitoring to ensure facilities actively participate in using the Anna platform.

Key responsibilities

• Monitor engagement metrics for assigned clients and prioritize outreach efforts based on facility performance
• Conduct outbound calls and email campaigns to facilities, onboarding new users and providing training on Anna workflows
• Validate engagement data, maintain accurate facility records, and prepare reports for leadership and clients

Required qualifications

• Three or more years of experience in provider relations, outreach, or care coordination within a healthcare setting
• Strong familiarity with post-acute care providers and their interactions with health plans
• Proven experience in cold outreach and building relationships with providers
• Proficiency in Microsoft Excel and Outlook
• Bachelor's degree required
