Palliative Care Nurse, RN

Job Summary

To support a growing virtual palliative care program, the full-time Palliative Care Nurse, RN will engage with patients through telephonic and video consultations, provide compassionate care coordination, and collaborate with interdisciplinary teams to enhance patient outcomes.

Key responsibilities

• Engage with patients to deliver home-based palliative care and conduct routine check-ins based on health status
• Perform care coordination and case management tasks, including collaborating with home health workers and communicating with primary care providers
• Participate in team huddles and palliative rounds, while supporting education initiatives for staff

Required qualifications

• Unrestricted RN licensure, with compact licensure preferred
• A minimum of 5 years of RN experience, including at least 2 years in hospice or palliative care
• CHPN credential or willingness to obtain within the first six months of employment
• Ability to work flexibly across time zones as needed
• Active Basic Life Support (BLS) certification
