Palliative Care Operations Manager

Job Summary

To support a growing home-based palliative care partnership, the full-time Operations Manager, Palliative Transition Partner will manage patient coordination, referral completion, scheduling, and reporting while building strong relationships with partner stakeholders in a remote environment.

Key Responsibilities

• Manage the team responsible for patient outreach and referral/screening completion
• Forecast staffing needs and report core central metrics for operational efficiency
• Hire, train, and manage central calling/scheduling staff while maintaining standard operating procedures

Required Qualifications

• 3+ years in healthcare operations, care coordination, or scheduling operations management, preferably in home-based care, hospice, or palliative settings
• Experience managing a team of coordinators/schedulers, including hiring and training
• Strong data/reporting skills with proficiency in Excel; AI skills are a plus
• Proven track record of managing operational rollouts or expansions across multiple sites
• Experience in relationship management, serving as a primary contact for external partners or clients
