Palliative Nurse Practitioner (NJ, PA, GA, VA, OK, KS)

Job Summary

To provide high-quality palliative care remotely, the full-time Palliative Nurse Practitioner (NJ, PA, GA, VA, OK, KS) will assess patients' health, develop care plans, and collaborate with care teams while fostering trust with patients and families.

Key responsibilities:

• Provide comprehensive palliative care to patients across multiple states
• Conduct physical examinations and create individualized care plans based on patient assessments
• Engage in sensitive discussions regarding goals of care and advance care planning with patients and families

Required qualifications:

• Board Certified with valid NP or PA certification in NJ, PA, GA, VA, OK, KS
• 2-3 years of experience in palliative, chronic care management, or primary care, preferably with geriatric patients
• Strong clinical skills for accurate assessment and diagnosis
• Positive demeanor and effective interpersonal skills
• Ability to operate independently and solve problems confidently
