Patient Access Coordinator

Job Summary

To support efficient patient admissions, the full-time Patient Access Coordinator will manage referral processes, oversee admission workflows, and coordinate with various teams while working remotely during night shifts.

Key responsibilities

• Initiates and manages patient referrals, ensuring accurate data entry into referral management platforms
• Coordinates admission logistics, including bed assignments and necessary paperwork, while confirming insurance verification
• Maintains the accuracy of the pending referral log and collaborates with the business development team to support patient admissions

Required qualifications

• High School Diploma or GED required; Associate's or Bachelor's Degree preferred
• Preference for candidates with a healthcare-related area of concentration or equivalent experience
• Previous experience in a healthcare-related role is preferred
• Experience in post-acute care is a plus
• Proficient in using standard and proprietary computer applications for data entry and management
