Patient Care Coordinator

Job Summary

Managing inbound and outbound referrals, the full-time Patient Care Coordinator will serve as the first point of contact for patients, coordinating care and scheduling while working remotely.

Key responsibilities

• Manage referral workflow by initiating outreach on unscheduled referrals and tracking outcomes
• Coordinate outbound referrals for patients needing services outside Talkiatry's scope, ensuring clear communication and follow-up
• Utilize motivational interviewing techniques to engage patients and assist with scheduling their psychiatric evaluations

Required qualifications

• 1-3 years of experience in healthcare coordination, patient intake, or referrals
• Experience in coordinating care across multiple organizations or provider networks is a plus
• Sales or customer retention experience strongly preferred
• Familiarity with CRMs, EHRs (eClinicalWorks), and ticketing systems (Zendesk/ServiceNow)
• Strong organizational skills and ability to multitask in high-volume environments
