Patient Care Coordinator

Job Summary

To support key business operations, the remote Patient Care Coordinator will focus on improving patient engagement, coordinating medication compliance outreach, and facilitating communication between patients and healthcare providers while working on a temporary contract basis.

Key responsibilities

• Coordinate outreach efforts to patients regarding medication compliance and preventive healthcare needs
• Support communication between patients, healthcare providers, and pharmacies to ensure timely access to care
• Maintain accurate documentation in electronic healthcare systems while ensuring compliance with data integrity requirements

Required qualifications

• High school diploma, GED, or equivalent required; Associate's or Bachelor's degree preferred
• Minimum of three years of experience in a healthcare environment, along with customer service experience
• Proficiency with Microsoft Office applications and electronic medical records
• Working knowledge of medical and pharmacy terminology and healthcare compliance practices
• Ability to type at least 45 words per minute
