Patient Care Coordinator

Job Summary

To support the Patient Coordination team, the part-time Patient Care Coordinator will conduct outreach calls to patients post-discharge, assist with non-clinical needs, and document patient information accurately while working remotely.

Key responsibilities

• Conduct outreach calls to ensure follow-up appointments and medication adherence for patients post-discharge
• Assist patients with navigating healthcare resources and referrals while documenting patient-reported concerns
• Collaborate with healthcare team members to ensure timely resolution of patient needs and maintain accurate records

Required qualifications

• Currently enrolled in an accredited college or university with at least sophomore standing
• Completion of at least one college-level health or science course related to healthcare concepts or medical terminology
• Customer service or patient-facing experience is a plus, with preference for those with experience as a Certified Nursing Assistant (CNA) or Medical Assistant (MA)
• Proficiency in Microsoft Office applications and collaboration platforms such as Teams or Webex
• Compassion and empathy in patient interactions, demonstrating professionalism and reliability
