Job Summary
To support patient-centered care, the part-time Patient Navigator will advocate for patients and their families, coordinate healthcare services, and connect individuals with community resources while working remotely.
Key responsibilities
- Advocate for patients and families, ensuring their needs are considered in care planning
- Serve as a liaison between patients, healthcare providers, and stakeholders to facilitate communication
- Collaborate with the healthcare team to coordinate care services and follow-up plans
Required qualifications
- Bachelor's Degree in Clinical Social Work, Healthcare Management, or a related field required
- 1-2 years of experience in social work, case management, or a related clinical setting preferred
