Job Summary
To support patient engagement and care navigation, the full-time Patient Navigator will assist patients in navigating the healthcare system by providing timely support for appointment scheduling, transportation coordination, and access to community resources in a remote environment.
Key responsibilities
- Connect with patients through various communication channels to develop rapport and encourage engagement in care
- Provide live support for appointment scheduling, transportation coordination, and medical record transfers while screening for social determinants of health
- Track patient needs to resolution, documenting interactions and escalating issues as necessary to ensure timely access to services
Required qualifications
- High School Diploma or G.E.D required; Associate's degree in healthcare or related field preferred
- Preferred training or certification as a Medical Assistant, Social Worker, Community Health Worker, or Patient Navigator
- Minimum of two years' experience in customer service, healthcare navigation, or patient support required
- Experience in care coordination or community resources preferred
- Ability to comply with HIPAA and other applicable laws during patient interactions
