Patient Navigator

Job Summary

To support patient engagement and retention, the full-time Patient Navigator will assist individuals in overcoming barriers to treatment access, including navigating insurance, financial assistance, and transportation resources, while primarily working in a remote setting with occasional in-clinic requirements.

Key responsibilities:

• Assist patients with Medicaid enrollment, renewal, and understanding insurance benefits related to treatment
• Educate patients on financial assistance options and coordinate support for transportation barriers
• Conduct outreach to re-engage patients who have missed appointments or are at risk of treatment interruption

Required qualifications:

• Associate's degree in Human Services, Healthcare Administration, or related field (preferred)
• Experience in healthcare, patient services, or community resources (preferred)
• Proficiency in insurance eligibility verification and benefit navigation
• Ability to determine patient financial responsibility for services
• Experience supporting underserved or behavioral health populations (preferred)
