Job Summary
Providing outreach to patients with complex medical and social needs, the full-time Pennsylvania Certified Community Health Worker will assist patients in navigating the healthcare system while working remotely.
Key responsibilities
- Orients and assesses new Community Health Workers and evaluates their proficiency during the training period
- Engages with patients through various means, including phone calls, home visits, and accompanying them to provider appointments
- Identifies gaps in care, connects members with resources, and facilitates adherence to treatment plans
Required qualifications
- High School Diploma or Equivalent (GED) - Required
- Minimum of 1 year of related work experience - Required
- Certified Community Health Worker (CCHW) certification from the Pennsylvania Certification Board - Required
