Job Summary
To support patient care coordination, the full-time salaried Pennsylvania Licensed Ambulatory Care RN will monitor outcomes, conduct telephonic outreach for clinical assessments, and ensure continuity of care while working remotely from Monday to Friday, 8:00 am to 4:30 pm.
Key responsibilities:
- Coordinate patient care by monitoring outcomes and maintaining communication with providers and payers
- Conduct telephonic outreach for clinical assessments and early symptom recognition
- Support patients' psycho-social and medical needs through age-appropriate interventions
Required qualifications:
- Minimum of two years recent experience in telephonic care management outreach
- ASN Degree required; Bachelor's Degree preferred
- RN License required
- Home care experience preferred
- Insurance or case management experience preferred
