Job Summary
To support members facing behavioral health challenges, the full-time Pennsylvania Licensed Behavioral Health Case Manager will engage clients remotely to develop personalized care plans, provide advocacy, and facilitate access to resources while collaborating with healthcare teams.
Key Responsibilities:
- Telephonically engage members to identify barriers to care related to Social Determinants of Health (SDOH)
- Develop care plans by assessing members' strengths, needs, and goals, providing follow-up as clinically appropriate
- Coordinate with healthcare teams to address complex behavioral health situations and ensure effective service delivery
Required Qualifications:
- Active PA Registered Nurse (BCBA preferred), LCSW, LSW, or LPC required
- Minimum of 3-5 years post-licensure experience in an integrated care or healthcare setting
- Strong understanding of Social Determinants of Health and Motivational Interviewing techniques
- Ability to work independently and resolve complex behavioral, medical, and psychosocial issues
- Exceptional assessment, engagement, and organizational skills with proficiency in Microsoft Office Suite
