Pennsylvania Licensed Behavioral Health Case Manager

Job Summary

To support at-risk members, the full-time Pennsylvania Licensed Behavioral Health Case Manager will provide telephone triage, crisis intervention, and care navigation for mental health and substance use concerns while working remotely in the Tri-State Area.

Key Responsibilities:

• Provides telephone triage, crisis intervention, and care navigation for at-risk members
• Engages effectively with members and families to assess needs and promote access to behavioral health services
• Matches members with appropriate providers and coordinates care with healthcare teams

Required Qualifications:

• Active PA Registered Nurse License or B.S.N., LSW, LCSW, LPC required
• Minimum of three to five years of post-licensure behavioral health clinical experience
• Experience in Utilization Review or Case Management preferred
• Knowledge of DSM 5 and ASAM
• Strong computer skills and experience with Microsoft Office Suite
