Job Summary
Coordinating clinical and financial plans for patients, the full-time Pennsylvania Licensed Care Manager will manage utilization, discharge planning, and post-acute care referrals while working remotely with a multi-disciplinary team.
Key responsibilities
- Evaluates patient records and applies InterQual criteria for care and authorization compliance
- Coordinates discharge planning and post-acute services with patients, providers, and community resources
- Collaborates with interdisciplinary teams to address barriers and optimize length of stay
Required qualifications
- Graduate of an approved school of nursing; BSN preferred
- Two years of nursing experience required
- Previous case management experience preferred
- Knowledge of healthcare financial and payor issues preferred
- Current licensure as a Registered Professional Nurse in Pennsylvania or a compact state
