Job Summary

To provide comprehensive health coaching and case management services, the full-time Pennsylvania Licensed Health Coach RN will work remotely, collaborating with members and healthcare teams to develop care plans, facilitate communication, and support self-management goals from health promotion to end-of-life care.

Key responsibilities:

• Functions as the primary nurse for members, offering support for lifestyle improvement and managing various health conditions
• Engages with members through coaching and intensive case management to enhance their self-management abilities
• Conducts assessments and develops care plans while monitoring progress and coordinating with healthcare providers as needed

Required qualifications:

• Current, active, and unrestricted Pennsylvania Registered Nurse licensure
• Three years of experience in clinical, case management, or disease/condition management roles
• Strong clinical knowledge relevant to health coaching
• Ability to work flexible hours
• Basic computer skills
