Job Summary
Joining a collaborative team, the full-time Pennsylvania Licensed RN Case Manager will assess, plan, implement, and evaluate patient care while working remotely to optimize health outcomes and coordinate services.
Key responsibilities:
- Assess healthcare, educational, and psychosocial needs of patients and design individualized care plans
- Integrate evidence-based clinical guidelines into treatment plans promoting quality and efficiency in healthcare delivery
- Continuously monitor and evaluate utilization patterns and quality metrics to enhance patient care outcomes
Required qualifications:
- Current Pennsylvania RN license
- Minimum of three years of nursing experience
- Preferred certification as a Case Manager from the American Case Management Association (ACMA)
- Bachelor's degree in a healthcare-related field is preferred
- Experience in case management is preferred
