Job Summary
Working remotely in Pennsylvania, the full-time salaried Pennsylvania Licensed RN Case Manager will assess, plan, implement, and coordinate patient-centered care while managing utilization and practice metrics to optimize health outcomes.
Key responsibilities
- Assess healthcare, educational, and psychosocial needs of patients to design individualized care plans
- Integrate evidence-based clinical guidelines in developing treatment plans that promote quality and efficiency
- Continuously evaluate diagnostic tests and utilization patterns to monitor quality and efficiency results for the assigned population
Required qualifications
- Current Pennsylvania RN license
- Minimum of two years of nursing experience
- Bachelor's Degree in Nursing preferred
- Certified Case Manager (ACMA) preferred
- Experience in case management preferred
