Job Summary

Working remotely in Pennsylvania, the full-time salaried Pennsylvania Licensed RN Case Manager will assess, plan, implement, and coordinate patient-centered care while optimizing health outcomes for members through effective case management practices.

Key responsibilities

• Assess and evaluate the healthcare and psychosocial needs of patients, developing individualized care plans in collaboration with the healthcare team
• Integrate evidence-based clinical guidelines into treatment plans to promote quality and efficiency in healthcare delivery
• Continuously monitor and adjust care plans based on laboratory results, diagnostic tests, and utilization patterns to ensure optimal patient outcomes

Required qualifications

• Current Pennsylvania RN license
• Minimum of two years of nursing experience
• Preferred certification as a Case Manager from the American Case Management Association (ACMA)
• Bachelor's Degree in Nursing (BSN) preferred
• Knowledge of healthcare laws, regulations, and insurance coverage policies is beneficial
