Pennsylvania Licensed RN Case Manager

Job Summary

To support a collaborative healthcare team, the full-time Pennsylvania Licensed RN Case Manager will manage patient-centered care coordination, assess and implement individualized care plans, and monitor health outcomes while working remotely.

Key responsibilities:

• Assess, plan, implement, coordinate, and evaluate options and services to optimize patient health status
• Develop and monitor systems of care that promote early intervention in acute care situations
• Maintain documentation for all case management activities and adjust treatment plans based on collected data

Required qualifications:

• Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania
• Minimum of 2 years of nursing experience
• Graduate from a Specialty Training Program in Nursing (Required), Bachelor's Degree in Nursing (Preferred)
• Certified Case Manager / American Case Management Association (ACMA) certification preferred
• Knowledge of healthcare laws, regulations, and insurance coverage policies is helpful
