Pennsylvania Licensed RN Case Manager
Job Summary
To support a growing health plan, the full-time salaried Pennsylvania Licensed RN Case Manager will assess, plan, implement, and coordinate patient-centered care while working remotely to optimize health outcomes for members.
Key responsibilities:
- Assess and evaluate the healthcare, educational, and psychosocial needs of patients or members
- Develop and implement individualized care plans in collaboration with multidisciplinary teams
- Monitor and analyze clinical metrics to enhance the quality and efficiency of care delivery
Required qualifications:
- Active Pennsylvania RN license
- Minimum of two years of nursing experience
- Preferred BSN or completion of a Specialty Training Program in Nursing
- Experience in case management is preferred, along with certification from the American Case Management Association (ACMA)
- Knowledge of healthcare laws, regulations, and insurance coverage policies is preferred
