Pennsylvania Licensed Service Coordinator

Job Summary

Supporting the coordination of long-term services and supports, the full-time Pennsylvania Licensed Service Coordinator will assist participants in navigating health services, conduct outreach activities, and ensure compliance with regulatory requirements while working remotely from Pennsylvania.

Key responsibilities:

• Establish and maintain strong relationships with participants and service providers to facilitate access to LTSS services
• Conduct outreach and provide telephonic support to participants, assisting with care management and service coordination
• Collect and analyze program data to track participant progress and ensure compliance with reporting requirements

Required qualifications:

• High School Diploma required
• 2 years of data entry or customer service experience required
• Preferred experience working with people with disabilities or seniors in need of LTSS
• Proficient in reading, writing, and communicating effectively in English
• Cultural competency and sensitivity to diverse backgrounds
