Pennsylvania Licensed Service Coordinator

Job Summary

To support the Community Health Choices program, the full-time Pennsylvania Licensed Service Coordinator will assist in providing service coordination services remotely, focusing on outreach, assessment, and coordination of long-term services and supports for participants in need of complex care management.

Key responsibilities

• Establish and maintain strong relationships with participants and partner providers to foster collaboration
• Assist participants in navigating and accessing long-term services and supports while ensuring compliance with regulatory requirements
• Collect and analyze program data to track participant progress and prepare reports regarding service provision

Required qualifications

• High School Diploma required
• 2 years of data entry or customer service experience required
• Preferred experience working with individuals with disabilities or seniors in need of long-term services and supports
• Ability to communicate effectively in English; additional language capability is preferred
• Demonstrated cultural competency and sensitivity to diverse backgrounds
