Pennsylvania Licensed Transition of Care Associate

Job Summary

To support patient engagement and care coordination, the part-time Pennsylvania Licensed Transition of Care Associate will interact with patients and families post-discharge, manage care gap reports, and collaborate with interdisciplinary teams in a remote setting.

Key responsibilities

• Conduct patient outreach and facilitate engagement with care management resources through telephonic and in-person visits
• Manage and update tracking systems for post-discharge care and care gap reports
• Collaborate with nursing teams to address barriers to care coordination and ensure effective patient engagement strategies

Required qualifications

• High School Diploma/GED with relevant healthcare experience or a specialized diploma in a related field
• At least 3 years of experience in a complex health system environment or less than 1 year with a specialized diploma
• Proficiency in Microsoft Office applications
• Knowledge of medical terminology and experience with electronic medical records
• Ability to maintain strict confidentiality and adapt to changing priorities
