Pharmacy Care Coordinator
Job Summary
Supporting clinical outreach and medication management, the full-time Pharmacy Care Coordinator will manage an assigned caseload, engage with members and providers, and facilitate care coordination remotely.
Key responsibilities
- Conduct outbound calls and utilize portal chat to schedule appointments for medication consultations
- Coordinate follow-ups with providers, pharmacies, and members to address medication recommendations and concerns
- Assist members in navigating their health benefits and participate in team activities and special projects
Required qualifications
- At least 3 years of work experience in health care or retail pharmacy
- Active National PTCB Certification
- Experience making a high volume of outbound calls to members, providers, and pharmacies
- Ability to manage daily responsibilities and tasks independently
- Flexibility to shift between tasks, projects, and priorities frequently
