Pharmacy Care Coordinator
Job Summary
Supporting clinical outreach and medication management, the full-time Pharmacy Care Coordinator will make outbound calls, manage a caseload, and coordinate care for members in a fully remote environment.
Key responsibilities
- Conducts outreach to schedule appointments with pharmacists and addresses medication concerns
- Manages assigned cases, ensuring timely follow-up and clear documentation
- Collaborates with the pharmacy team and participates in special projects to enhance member care
Required qualifications
- At least 3 years of work experience in health care or retail pharmacy
- Active National PTCB Certification
- Experience making a high volume of outbound calls to members, providers, and pharmacies
- Ability to manage daily responsibilities and tasks independently
- Comfortable adapting to shifting tasks and priorities
