Population Health Care Coordinator
Job Summary
To support a growing healthcare practice, the full-time Population Health Care Coordinator will manage patient data, schedule appointments, and ensure effective communication regarding care needs while working onsite in Layton, UT.
Key responsibilities
- Review patient data to identify and schedule Annual Wellness Visits and other care needs with providers
- Make outbound scheduling calls and conduct patient outreach via targeted email campaigns
- Support providers in improving medication adherence and reducing unnecessary ER utilization through patient education and follow-up visits
Required qualifications
- High School diploma or equivalent
- Experience as a central scheduling representative preferred; familiarity with medical terminology is helpful
- Proficient working knowledge of Microsoft Suite and other computer programs
- Ability to maintain a professional demeanor in stressful situations
- Regular and reliable attendance is essential
