Population Health IT Analyst
Job Summary
To support the organization's Population Health strategy, the full-time remote Population Health IT Specialty Analyst will manage the day-to-day support, optimization, testing, and maintenance of Epic Population Health applications while collaborating with clinical, operational, and technical teams.
Key responsibilities
- Support and optimize Epic Population Health applications, including Inpatient Case Management and Healthy Planet
- Analyze workflows, evaluate software releases, and resolve application-related issues in partnership with cross-functional teams
- Assist in application integration, system upgrades, and ongoing operational support to enhance patient outcomes and clinician efficiency
Required qualifications
- Bachelor's Degree in Nursing, Health Information Management, Healthcare Administration, Information Technology, or a related field, or 5 years of relevant experience in lieu of a degree
- 3 years of relevant healthcare, clinical informatics, application support, or IT analyst experience
- Experience in clinical case management, care coordination, or population health programs
- Knowledge of inpatient nursing workflows and healthcare operations
- Certification in Epic HP (Healthy Planet) or IP ClinDoc preferred
