Key responsibilities

- Support the Care Coordination Program across all SLPG Primary Care providers and clinics
- Collaborate with stakeholders to maximize performance within value-based contracts
Required qualifications
- Less than 1 year of experience as a Medical Assistant (Certified)
- Associate Degree in a relevant field
- Understanding of population health management and value-based care principles
