Job Summary
Connecting with patients through daily video check-ins, the per-diem Puerto Rico Licensed Clinical Nurse will provide encouragement, education, and accountability to support medication adherence and improve health outcomes in a remote setting.
Key Responsibilities:
- Conduct patient outreach and engagement through video messaging, consultations, and phone calls to educate and enroll patients in the adherence program
- Utilize video-based communication to deliver personalized education, coaching, and adherence support
- Provide compassionate coaching and health education services to encourage medication adherence and lifestyle changes
Required Qualifications:
- Must be a legal resident of Puerto Rico
- Registered Nurse (RN) license in good standing with at least 5 years of nursing experience
- 5 years of related experience in patient engagement, health education, or care coordination
- Fluency in both English and Spanish, with the ability to read, write, speak, and understand both languages proficiently
- Strong background in chronic disease management, medication adherence, and patient coaching
