Puerto Rico Licensed Clinical Pharmacist
Job Summary
To support a growing healthcare initiative, the part-time Puerto Rico Licensed Clinical Pharmacist will conduct Comprehensive Medication Reviews (CMRs), establish protocols for patient care, and collaborate with a multidisciplinary team to enhance medication adherence through remote engagement.
Key responsibilities
- Build trust-based relationships with patients through telephonic and video engagement
- Deliver Medication Therapy Management (MTM) services, including CMRs and medication reconciliation
- Proactively address social determinants of health affecting patient adherence and connect them to appropriate resources
Required qualifications
- Must reside in Puerto Rico
- Active Pharmacist License with at least 3 years of pharmacy practice
- Medically fluent in English and Spanish
- Demonstrated experience in patient relationship-building roles such as health coaching or chronic disease management
- Comfort with motivational interviewing and behavioral health screening techniques
