Job Summary
Working in a hybrid capacity, the full-time Quality Care Coordinator will improve health outcomes for patients by addressing preventive health measures, enhancing care retention, and collaborating across departments to remove barriers to care.
Key responsibilities
- Provide outreach to patients and support the Quality Care Management nursing team
- Address gaps in care and improve HEDIS and Ryan White Standards outcomes
- Engage in telephonic interactions with patients to advocate for their health and empower them in their care journey
Required qualifications
- Strong knowledge of Healthcare Effectiveness Data and Information Set (HEDIS)
- Familiarity with HIV/AIDS Bureau Ryan White Standards
- Experience in healthcare or patient advocacy
- Ability to work collaboratively in a team-oriented environment
