Quality Improvement Specialist

Job Summary

To support the improvement of kidney care, the remote Quality Improvement Specialist will manage patient and provider panels, deliver actionable performance insights, and drive improvements in HEDIS and Medicare Stars measures through targeted education and collaboration.

Key responsibilities

• Engage with providers to educate on HEDIS and Stars expectations, documentation standards, and performance improvement strategies
• Manage patient outreach efforts to close care gaps and coordinate care, ensuring effective communication between patients and providers
• Collaborate with cross-functional teams to support quality initiatives and maintain compliance with regulatory standards

Required qualifications

• 1-2 years of experience in quality improvement, population health, provider engagement, or care coordination
• Experience with patient-facing outreach and understanding of quality workflows and documentation requirements
• Proficiency in EMR systems and healthcare technology tools
• Intermediate skills with MS Office Suite, including Outlook and Teams
• Able to work effectively in a primarily remote environment with a suitable internet connection
