Referral Navigator

Job Summary

Serving as the main point of contact for the agency's EMR/Referral portals, the full-time Referral Navigator will manage prompt responses to referral requests while demonstrating professionalism and excellent customer service in a remote setting.

Key responsibilities:

• Manage EMR/Referral portals in accordance with Referral Acceptance Guidelines
• Provide timely responses to referral requests and communicate with referral sources
• Conduct pre-checks on patient insurances and escalate referrals requiring clinical approval

Required qualifications:

• Associate's degree preferred
• At least one year of experience in home health and customer service
• Proficiency in managing EMR portals and Microsoft Office suite
• Familiarity with medical terminology is preferred
• Strong data entry skills and ability to work efficiently in a fast-paced environment
